In some patients, the milk ducts behind the nipple are too short, causing the nipple to retract. This retraction is often bothersome for aesthetic reasons and because it can prevent breastfeeding in patients who wish to do so. A breast examination including a mammogram and ultrasound is essential prior to surgery. An inverted nipple can be treated surgically to restore its projection and shape.
The procedure is performed under local anaesthetic. A 5 mm incision is made at the base of the nipple, which is then lifted. The milk ducts that are too short are then severed and stitches are placed to keep the nipple in a non-invaginated position. These stitches will be left in place for 4 weeks to prevent the nipple from sagging again. Post-operative pain is minimal and can usually be relieved with paracetamol.
