In some patients, the milk ducts behind the nipple are too short, leading to nipple retraction. This retraction is often bothersome, for aesthetic reasons on the one hand, and because it may prevent breast-feeding in patients who wish to do so. A mammogram and ultrasound are essential prior to surgery. Invaginated nipples can be surgically treated to restore nipple projection and relief.
The procedure is performed under local anaesthetic. A 5 mm incision is made at the base of the nipple, which is lifted. The shortened milk ducts are then cut, and sutures are inserted to hold the nipple in the uninvaginated position. These stitches are left in place for 4 weeks to prevent further nipple collapse. Post-operative pain is slight and generally relieved by paracetamol.
